In order to schedule a new patient appointment, please provide the following information for our staff and allow 24 business hours for our staff to contact you for an appointment.

Cancellation Policy: Your method of payment on file will be automatically billed $85.00 for a missed appointment fee if you are more than 10 minutes late for your appointment or no show for you appointment.

Our office is open! We are excited announce that as of January 15, 2022 we are able to offer telehealth services to our existing patients. If you prefer an in office appointment, we are asking that clients wear masks if there has been a recent illness or exposure to someone who is ill. Please do not present to our office if you have been ill within the last 48-72 hours. Thank You!
MEDICATION REFILL REQUEST FORM
If you are requesting medication refills…please use this form and it will be emailed to your provider and their clinical team for completion. Please allow 24 business hours for your medications to be available at your pharmacy.
Please note that as of May 2022, there has been a national Adderall and Ritalin shortage. We encourage patients to contact your pharmacy to check the availability of these medications prior to requesting a refill from our office.
This shortage is out of the control of our office, your provider and your pharmacy as it is a manufacturer and distribution issue. We are unsure when this will resolve and are being kept up to date by the Food and Drug Administration (FDA) daily on this issue. We appreciate your cooperation and we understand that this is an inconvenience for our patients.
Please note that medications will only be filled until your next appointment.
For example, if you have an appointment in 1 month, a 30 day supply of medications will be refilled.
Controlled medications such as Adderall and Ritalin products, will only be called in for a 30 day supply due to the national shortage. It is your responsibility to notify our office when your Adderall or Ritalin products are due to be refilled.
