Disability Evaluations
Disability is a difficult word.
When a client becomes disabled from a mental health condition, it can be difficult to have the documentation to back up the diagnosis.
The providers at The Journey Behavioral & Wellness Clinic are experienced at completing disability evaluations and follow up paperwork for mental health disability claims.
We do not accept or file insurance. In order to help our clients receive reimbursement for services, we do provide a form at the end of each month that can be submitted to your insurance company for reimbursement purposes.
Our Policies:
Our clients must establish and maintain mental health counseling for disability evaluations/follow up paperwork to be completed.
Initial disability forms are $100 for the complete packet and $65 for follow up paperwork to be completed. These fees must be paid prior to release of disability paperwork.
Release forms must be signed from therapist/mental health counselor for continued communication.
Our clients must maintain and attend three (3) consecutive appointments with their provider in order to have disability paperwork completed. If appointments are missed, our providers will not be able to complete paperwork.
Disability forms will need to be completed in your visit, so accurate information is provided to the appropriate parties involved in your claim.
